Abstract

A 26-year-old female with a 13-year history of ulcerative colitis (UC). In 2012, she developed recurrent abdominal pain with mucus-blood stool without apparent cause. Treatment with conventional aminosalicylates, corticosteroids, biologics (vedolizumab, infliximab, upadacitinib, and etc), and leukocyte adsorption yielded suboptimal results. While drug induction periods provided partial symptom relief, endoscopic performances revealed persistent inflammatory activity of varying degrees. Pre-medication endoscopic image of guselcizumab revealed marked distortion of microarchitecture, indicating significant histological inflammatory activity.
