Toward safer rechallenge after severe immune-mediated hepatotoxicity: the need for dynamic and mechanism-informed risk stratification
Abstract
We read with great interest the study by Pang and colleagues, which offers timely real-world evidence on severe immune-mediated hepatotoxicity (IMH) following immune checkpoint inhibitor (ICI) treatment [1]. Drawing on 408 patients from two high-volume Chinese centers, the authors identified hepatitis B virus (HBV) infection, liver malignancy, cholelithiasis, and elevated lactate dehydrogenase (LDH) as independent risk factors for severe IMH. These findings are clinically meaningful, especially in HBV-endemic areas, where oncologists are often required to balance anticancer efficacy against the possibility of liver-related harm.
