Rituximab and Cycle-Synchronized Prednisone for Immune Checkpoint Inhibitor Rechallenge in Minimal Change Disease
Abstract
Immune checkpoint inhibitors (ICI) – including cytotoxic T-lymphocyte antigen-4 (CTLA-4), programmed cell death protein-1 (PD-1) and programmed death-ligand 1 (PD-L1) inhibitors – have transformed the treatment of advanced malignancies. However, their immune activating mechanisms reduce self-tolerance and cause renal immune related adverse events (irAEs) in 2-5% of patients, limiting continuation of life-saving therapy.1 Corticosteroids remain the cornerstone of irAE management, yet prolonged steroid use suppresses the cytotoxic T-cell response that underlies ICI efficacy and has been associated with worse survival outcomes in ICI recipients.
