Abstract

Genetic cancer predisposition syndromes (CPS) significantly increase an individual’s risk of developing cancer, oftentimes at young ages. Syndrome-specific cancer screening regimens have been created to identify these cancers at early stages. This screening may be life-long, and as these patients age, they must undergo a meaningful transition from child-centered to adult-oriented healthcare to maximize the likelihood of medical follow-up. To improve continuity of care, many specialized pediatric clinics are developing formal transition programs that guide adolescents and young adults (AYAs) into adult healthcare.
