Abstract

Cardiac rehabilitation improves survival and quality of life, yet real-world participation and long-term adherence remain suboptimal. Digital telerehabilitation delivered by nurses can extend reach and continuity across primary care and community settings, but durability beyond the typical 6–12-month evaluation window is uncertain. We report a long-term follow-up of a randomised trial testing whether a nurse-led eHealth cardiac rehabilitation program sustains telemonitoring engagement and improves device-measured physical activity 17 months after completion compared with usual care.
