Abstract

Hospital bed management requires optimizing occupancy levels while maintaining sufficient surge capacity to prevent patient boarding and extended wait times. Industry guidelines target 80-85% occupancy, as levels beyond 90% create patient waits and diversions. Traditional operations-improvement methods have provided incremental gains, while recent data-driven tools and command centers offer new optimization potential. This study synthesizes insights from global case studies and institutional interviews to evaluate how integrated Lean principles and artificial intelligence approaches optimize hospital bed capacity management across diverse healthcare settings. We analyzed implementations using the 4S framework (Statement, Selling Point, Summary, Sources), reviewing Lean methods (Kanban, Andon, push-pull flows) alongside AI-enabled scheduling models (LeanTaaS iQueue, Qventus) and command-center platforms. Analysis covered multiple departments including emergency, inpatient, operating rooms, laboratories, and pediatrics, with empirical outcomes assessed qualitatively and quantitatively. Combined Lean-AI approaches demonstrated substantial improvements in wait times, length of stay, and effective capacity. Command center implementations achieved equivalent capacity gains of 30+ beds without physical expansion. Key enabling factors included real-time data visibility, organizational culture supporting change, and interdepartmental coordination. Implementation challenges centered on data reliability and return-on-investment measurement. Hospitals can address rising demand through three strategic priorities: investing in real-time visibility infrastructure (dashboards, location tracking), adopting flexible workflows (Lean huddles, automated alerts), and deploying advanced forecasting using machine learning. Success requires integrating technological solutions with operational process improvements rather than implementing either approach independently.
