Abstract

Agentic AI systems capable of goal-directed planning, tool use, reflection, and multi-step decision-making represent a paradigm shift from passive chatbots to proactive digital collaborators in healthcare. Globally, aging populations, clinician workforce shortages estimated at 5.9 million nurses by the World Health Organization, and uneven resource distribution strain health systems across high, middle, and low-income countries. This paper examines the technological foundations, real-world implementations, regulatory enablers, clinical impacts, ethical challenges, and future trajectories of agentic AI in health consultation worldwide. Drawing on peer-reviewed literature, market analyses, regulatory documents, and case studies from North America, Europe, Asia-Pacific, and emerging economies, we argue that while technological convergence positions agentic AI as a transformative force, robust governance frameworks are essential to ensure safety, equity, and trust. The global agentic AI in healthcare market, valued at approximately USD 0.79 billion in 2025, is projected to reach USD 33.66 billion by 2035 (CAGR 45.6%), with multi-agent systems and ambient clinical intelligence emerging as the fastest-growing segments. This synthesis of evidence from 2020–2026 identifies critical gaps in clinical validation, algorithmic bias mitigation, and regulatory harmonization that must be addressed to realize the full potential of agentic AI in democratizing healthcare access.
