Anesthetic considerations for therapeutic whole-lung lavage in acquired pulmonary alveolar silico-proteinosis: A case report
Abstract
Pulmonary alveolar proteinosis is a rare disorder characterized by intra-alveolar surfactant accumulation resulting in severe impairment of gas exchange. Whole-lung lavage (WLL) remains the definitive therapy but presents significant anesthetic challenges, including lung isolation, risk of hypoxemia, large-volume fluid instillation, and hemodynamic instability. We describe the anesthetic management of a 28-year-old male with pulmonary alveolar silico-proteinosis scheduled for therapeutic WLL. Meticulous planning and multidisciplinary coordination enabled safe completion of the procedure with successful postoperative recovery and extubation.
