A Challenging Pulmonary Embolism: When Acute Meets Chronic Thrombus
Abstract
A 24-year-old man with refractory hypercoagulability (not otherwise specified) was admitted for repeated high-risk pulmonary embolism (PE). His first presentation years ago required veno-arterial extracorporeal membrane oxygenation (VA-ECMO) support followed by pulmonary thromboendarterectomy (PTE). Recently, while on warfarin, he developed another large saddle PE with right ventricular (RV) thrombus. Treatment with heparin resulted in massive hemoptysis, and ultimately, he underwent a second PTE with RV thrombus removal.
