Abstract

We thank Hofmann and Dagi1 for their thoughtful and substantive response to our work and for expanding the discussion to include the formidable economic, regulatory, and incentive-based challenges that confront surgeon-innovators. Their articulation of the “valley of death” between proof-of-concept and patient impact is accurate, well supported, and reflective of the lived experience of many clinicians attempting to translate ideas into therapies. We fully acknowledge the magnitude of these financial and structural barriers.
