Abstract

ABSTRACT This is a case of a 15-year-old male with a congenital hyperpigmented lesion on the nasal tip and columella, which increased in size as the patient grew. A childhood consult and preoperative assessment were performed; however, the patient was lost to follow-up. During adolescence, he again sought consultation for removal of the lesion, which by then occupied multiple nasal subunits. A bilobed rotational flap of the nasal tip and a medial lip advancement flap were performed in a single operation to reconstruct the affected area. The use of these techniques allowed preservation of the philtrum and other important aesthetic units of the face. The patient not only tolerated the procedure well but was also prepared to face life—head up, loud, and proud.
