Abstract

Inaccurate allergy documentation in Electronic Medical Records (EMR) compromises patient safety in Emergency Department (ED), restricting first-line therapies and increasing costs. Clinical Pharmacists (CPs)-led allergy verification in ED setting remains underexplored. This study aims to assess discrepancies between EMR-documented allergies and patient self-reports using CPs-led structured allergy verification process. Findings led to a quality improvement initiative to address gaps.
