Abstract

Hypertension is common in individuals with hypertrophic cardiomyopathy (HCM); however, its optimal management has not been elucidated. Treatment with thiazide diuretics, angiotensin converting enzyme inhibitors (ACEi), angiotensin receptor blockers (ARBs), and dihydropyridine calcium channel blockers (DHP-CCBs), may worsen left ventricular outflow tract (LVOT) obstruction and symptoms.1 We previously demonstrated that blood pressure (BP) rises with LVOT gradient reduction in patients on mavacamten, a first-in-class cardiac myosin inhibitor (CMI), and remains elevated compared with baseline BP.
