Abstract

For eligible myelofibrosis (MF) patients, allogeneic hematopoietic stem cell transplantation (alloHSCT) remains the only curative treatment. Based on possible better transplant outcomes in responding patients [ 1 ], current EBMT/ELN-guidelines recommend pre-transplant spleen-directed therapy with JAK-inhibitors, but also emphasize the unmet need of data on the use of newer generation JAK-inhibitors in this setting [ 2 , 3 ]. Transplant-eligible MF patients were excluded from all landmark JAK-inhibitor trials. The single-arm, non-randomized HOVON-134 trial specifically aimed to answer this question: Is the use of the JAK1-sparing JAK2/IRAK1/ACVR1-inhibitor pacritinib (PAC) feasible in transplant-eligible MF patients, particularly with regards to spleen and symptom responses, safety and patients’ ability to proceed towards intended alloHSCT.
