Abstract

Abstract: Cochlear implant (CI) care in Germany is largely standardized through the national CI clinical practice guideline, the CI White Paper, and established certification procedures. Lifelong aftercare is essential for maintaining long-term treatment outcomes. In particular, technical aftercare may, in accordance with current guidelines, be delegated by a cochlear implant providing department (CIVE) to qualified external cooperation partners (QEK). To preserve the CIVE's overall responsibility, a clearly structured framework for collaboration is required. The aim of this manuscript is to present a practical model for the guideline-compliant delegation of technical aftercare services.A structured cooperation model was developed based on practical experience from existing collaborations and with consideration of the CI clinical practice guideline, the CI White Paper, and applicable regulatory requirements. The model was derived from cooperation agreements currently implemented at the authors' institution with various QEKs. It includes delegable and non-delegable services, qualification requirements, communication and documentation processes, escalation criteria, and quality assurance measures. The structured delegation of technical aftercare to qualified external partners represents a practical and guideline-compliant model for quality-assured CI aftercare. Prerequisites are clearly defined delegation limits, binding quality standards, and continuous supervision by the CIVE. Furthermore, the integrated communication and documentation framework may serve as a basis for future cross-sector and telemedical care structures.
