Abstract

Background: Depemokimab is the first ultra-long-acting biologic with enhanced interleukin-5 binding affinity, high potency, and extended half-life, enabling twice-yearly dosing in patients with asthma. In SWIFT-1/2, depemokimab significantly reduced exacerbations with sustained suppression of inflammation assessed by blood eosinophil count in patients with type 2 asthma. CRSwNP is a common asthma comorbidity, also often driven by type 2 inflammation. Aims: Evaluate depemokimab efficacy in reducing annualised exacerbation rate in the CRSwNP subpopulation from SWIFT-1/2. Methods: Patients were randomised 2:1 to receive depemokimab 100 mg subcutaneously or placebo, plus standard of care, once every 26 weeks over 52 weeks. Annualised exacerbation rate was analysed in patients with past or current CRSwNP at baseline at Week 52 (pre-specified). Results: At Week 52, among CRSwNP subpopulation, the annualised exacerbation rates were 0.51 (95% confidence interval [CI]: 0.33, 0.77; n=80) for depemokimab and 1.70 (95% CI: 1.02, 2.82; n=33) for placebo, with the depemokimab versus placebo rate ratio of 0.30 (95% CI: 0.15, 0.58; p
