Extracranial-Intracranial Bypass with Radial Artery Graft for Basilar Artery Ischemia
Abstract
We describe the surgical management of a patient with intractable basilar ischemia with repeat 80% artery stenosis at the right vertebrobasilar junction. A complex EC/IC high-flow bypass design utilized a radial artery graft to the P2 segment of the posterior cerebral artery via a right orbitozygomatic craniotomy with anterior clinoidectomy. Distal and proximal anastomoses were made under burst suppression and systemic heparinization. Doppler and indocyanine green angiography confirmed graft patency and robust flow augmentation.
