Abstract

We present the case of a 72-year-old female with retroorbital pain, proptosis, and right-sided optic neuropathy from a spheno-orbital meningioma with cavernous sinus extension, hyperostosis, and intraorbital involvement. An endoscopic transorbital approach enabled tumor and bone removal, including anterior clinoidectomy, followed by optic nerve decompression from cisternal to intraorbital segments. The patient recovered uneventfully, and at 3 months showed marked visual improvement, underscoring this approach’s ability to address the optic nerve along its full course.
