Tumor Removal of a Petroclival Meningioma with Severe Brainstem Edema and Hydrocephalus Through Subdural Subtemporal Anterior Transpetrosal Approach
Abstract
A 58-year-old woman presented with blurred vision and unsteady gait for over 6 months. MRI showed a lobulated left petroclival meningioma with significant brainstem edema, dislocation, and supratentorial hydrocephalus. Due to severe edema and T2 low signal (firm consistency), subtotal resection was performed via a subdural subtemporal anterior transpetrosal approach (experience >1000 cases). Postoperatively, hydrocephalus alleviated; pathology revealed transitional meningioma with brain infiltration, no CSF leak, and unchanged neurological function.
