Contralateral Transcallosal-Transchoroidal Approach with Endoscopic Assistance for Resection of a Large Thalamo-Mesencephalic Cavernoma
Abstract
A 14-year-old female presented with progressive left-sided weakness, tremor, dysphagia, and gait disturbance due to a right thalamo-mesencephalic cavernoma. After an initial partial resection, a redo contralateral transcallosal transchoroidal approach with endoscopic assistance was performed. The suprachoroidal opening enabled safe access, and meticulous microsurgical and endoscopic dissection achieved complete excision. Postoperative MRI confirmed total removal and relief of mass effect. This case highlights technical strategies for managing cavernomas in deep, eloquent brain regions. Publication History Article published online: 27 February 2026 © 2026. Thieme. All rights reserved.
