Abstract

This operative video presents a 62-year-old female with progressive proprioceptive deficits and imbalance due to a 2.5 cm right thalamic cyst protruding into the third ventricle. A frontal transcortical endoscopic approach with endoscopic third ventriculostomy was performed. Fenestration was achieved using blunt dissection, Fogarty balloon dilation, and enlargement with alligator clamps. Postoperatively, the patient experienced complete resolution of symptoms with radiographic improvement, demonstrating that endoscopic fenestration can safely and effectively treat deep symptomatic thalamic cysts.
