Minimally Invasive Parafascicular Approach for Visualization and Cauterization of Bleeding Charcot Bouchard Aneurysm
Abstract
Charcot-Bouchard aneurysms are historically linked to hypertensive hemorrhage but rarely visualized intraoperatively, leaving debate over their causal role. We present direct visualization and cauterization of an actively bleeding Charcot-Bouchard aneurysm during minimally invasive parafascicular surgery. A patient with hypertension and diabetes presented with acute aphasia and hemiparesis from a large sub-insular hemorrhage. Emergent tubular retractor-guided evacuation enabled hemostasis. This operative video highlights minimally invasive management of hypertensive hemorrhage and supports microaneurysm rupture in its etiology.
