Abstract

The surgical management of large middle cerebral artery (MCA) aneurysms presents a challenge. In this video, we display the use of high-flow bypass from the common carotid artery (CCA) with trapping of a right M1 aneurysm in a 45-year-old female with a high CCA bifurcation. Pterional craniotomy was used for the distal anastomosis and aneurysm trapping. Neck dissection allowed for proximal anastomosis of the radial artery graft. Trapping and bypass allowed for aneurysm occlusion. Publication History Article published online: 27 February 2026 © 2026. Thieme. All rights reserved.
