Abstract

Bimekizumab is a humanized monoclonal antibody to interleukins-17A and -17F, licensed in 2023 for the treatment of moderate to severe hidradenitis suppurativa (HS). This retrospective case series discusses 15 patients who were treated with bimekizumab for HS. Of the total, 74% were female. The mean age of the patients was 42 years; 87% had Hurley stage III HS and the mean baseline International Hidradenitis Suppurativa Severity Score System score was 20.13. All were biologic experienced and 11/15 were taking concomitant medications for the treatment of HS. At 16 weeks, as calculated by HS Clinical Response or HS Physician Global Assessment, 10/15 (67%) patients responded to bimekizumab. The mean Dermatology Life Quality Index fell by nine points. One patient experienced secondary failure at 9 months. Response rates in this group at week 16 were higher than those seen in phase III clinical trials, despite previous biologic exposure in all patients. While concomitant HS medication may play a role in this higher response rate, bimekizumab is an important treatment option for moderate to severe HS.
