Abstract

Abstract Introduction Penile fracture is caused by the rupture of the tunica albuginea during an erection. In some cases, an associated urethral injury may occur, increasing the risk of complications. Factors such as the trauma mechanism, particularly the sexual position, may influence the severity of the injury. This study aims to evaluate whether there is an association between the mechanism of penile fracture during sexual intercourse and the urethral injury. Methods A retrospective, descriptive, and analytical study included 84 cases treated between 2007 and 2023 at a university hospital. Age, trauma mechanism, presence of urethral injury, and bilateral lesions were evaluated. The association between the fracture mechanism and the presence of urethral and bilateral injuries was analyzed. Statistical analysis used the Chi-squared test in R statistical software, adopting a significance level of p < 0.005. Results A total of 84 men were evaluated, with a mean age of 35.3 (±11.6) years. The mechanism of injury was sexual intercourse in 76.2% (n = 64) of cases, and other causes accounted for 23.8% (n = 20). Among patients with penile fracture during sexual intercourse, 16 had associated urethral injury, while only 1 patient from the non-sexual trauma group presented with urethral injury (p = 0.121). Statistical analysis showed no significant association between the trauma mechanism and urethral injury (p = 0.419). Regarding sexual positions, penile fracture occurred in 57.9% of cases during doggy style position, 31.6% during man-on-top, and 10.5% in other positions. No significant association was found between sexual position and urethral injury (p = 0.467). Conclusion Penile fracture occurs predominantly during sexual intercourse. Although urethral injury was observed in some patients with sexually related trauma, statistical analysis did not demonstrate a significant association between the trauma mechanism and the occurrence of this complication. Financing No conflict.
