Abstract

Abstract Introduction In cases where traditional sperm retrieval methods such as TESA and mTESE fail to obtain sperm, sperm bank donation becomes an alternative treatment option for NOA patients. However, there is a lack of research regarding the proportion of NOA patients who choose sperm bank donation during treatment and the factors influencing this decision. Objective This study aimed to analyze the proportion of NOA patients selecting sperm bank donation under different sperm retrieval methods based on five years of data from Peking University Third Hospital’s Reproductive Medicine Center. Methods We collected clinical data of 2132 NOA patients who underwent in vitro fertilization (IVF) at Peking University Third Hospital from January 2019 to December 2023. Patients' age, BMI, sperm retrieval methods (mTESE, TESA, frozen mTESE, frozen TESA), and sperm source (sperm from the patient or sperm bank) were analyzed. A log-linear model was used to evaluate the relationship between sperm retrieval methods and sperm source. Inclusion criteria included male patients diagnosed with NOA, while exclusion criteria involved patients with other fertility-compromising conditions or missing data. Results Among the 2132 NOA patients, 11.82% opted for sperm bank donation. Specifically, the sperm bank usage rate was highest in the mTESE group (21.17%), followed by TESA (8.11%), frozen mTESE (4.33%), and frozen TESA (0%). Log-linear model analysis showed a significant negative correlation between sperm retrieval methods (mTESE, P
