Intensive vs. Moderate-Intensity Statins After Acute Coronary Syndromes
Abstract
Abstract This chapter appraises the PROVE IT-TIMI 22 trial, which compared intensive and moderate-intensity statin therapy following acute coronary syndrome (ACS) to determine their impact on cardiovascular risks. PROVE IT-TIMI 22 recruited 4162 patients and showed that high-dose atorvastatin significantly reduces the primary endpoint of death, myocardial infarction (MI), stroke, and revascularization by 16% compared to pravastatin. Intensive therapy also reduced the risk of coronary heart disease death, MI, and urgent revascularization by 14%. Despite these benefits, the study found a higher rate of adverse effects, including myalgias and liver function abnormalities, leading to drug discontinuation in some patients. Finally, the chapter considers the overall benefits high-dose atorvastatin, particularly for ACS patients with elevated low-density lipoprotein levels, to reduce cardiovascular risks.
