4Ts Score to Exclude Heparin-Induced Thrombocytopenia
Abstract
Abstract This chapter examines the 4Ts score as a tool for excluding heparin-induced thrombocytopenia (HIT) in patients with thrombocytopenia. The underlying study compared the score’s negative predictive value in two cohorts: one from Canada and another from Germany and Austria. The chapter recapitulates that a low 4Ts score (0 to 3) correlates with a very low rate of positive HIT assays, suggesting that further testing is unnecessary in these patients. The evidence supports using the 4Ts score to guide clinical decisions, particularly when patients have a low likelihood of HIT. Finally, the chapter emphasizes that while the 4Ts score is effective in excluding HIT, clinicians should consider additional testing for intermediate or high scores.
