Patient-Targeted Discharge Education to Prevent Heart Failure Readmissions
Abstract
Abstract This chapter examines the impact of patient-targeted discharge education on reducing heart failure readmissions. A research team at the University of Michigan Hospital conducted a randomized controlled trial to evaluate the effect of 1-hour, individualized chronic heart failure education provided by nurse educators to 223 patients with heart failure and left ventricular systolic dysfunction. The trial demonstrated that the education group experienced fewer rehospitalizations and lower mortality rates within 180 days compared to the control group. However, researchers observed no significant difference in mortality rates between the two groups. Finally, the chapter addresses the study’s limitations, such as its single-site nature and uncertainties regarding how health literacy or language proficiency influenced the outcomes.
