Behavioural Problems in Developmental Disorders
Abstract
Abstract This book outlines a comprehensive paediatric perspective on challenging behaviours in children with developmental disabilities. Recognizing that standard medical training often falls short in this area, the text provides a structured approach, starting with the role of paediatricians in assessment and ongoing care. It explores the multifaceted causes of behaviour problems, considering disability-specific, health, psychological, environmental, and historical contributions. Clinical cases are used throughout reflecting issues discussed. In addition to consideration of what to assess, the book discusses consultation strategies and how to objectively evaluate contributing causes as the foundation for assessing outcomes. This information is assembled into a behavioural diagnostic formulation. For this, a 5P model (predisposing, protective, precipitating, perpetuating, and palliating factors) is recommended and discussed as a practical and comprehensive alternative to traditional diagnoses. This model integrates individual developmental information into an explanation of behaviour causation that can serve as the basis for management planning and evaluation. Management strategies begin with safety and child health. A broad range of interventions are considered alongside evaluable treatment outcomes, against which efficacy can be reviewed. In addition to what the paediatrician can do alone, the book discusses optimal collaboration with family, other professionals and community figures. Finally, the text explores the behavioural profiles (phenotypes) associated with various developmental disorders, including genetic syndromes, epilepsy, ADHD, autism, and intellectual disability, providing in-depth guidance for common paediatric consultations. This resource aims to empower clinicians with the knowledge and tools necessary for effective management of problem behaviour in this vulnerable group of children.
