Dissecting the Fissure
Abstract
Abstract A “flipping” technique is described to assist with safe visualization that is specific to the anatomy of each lobectomy. Fissure-less lobectomy can usually be safely performed when no intervening bronchus is present to obstruct the staple line. Other clinical factors, such as thick fissures, inflammation, desmoplasia, aberrant anatomy, asbestosis, or the need to check for resectability in advanced disease can all complicate a fissure-less approach.
