Abstract

We read with great interest the article by Lowe et al., “Medical Direction Needs Analysis,” which aptly addresses the challenges related to preparing physicians for leadership roles in prehospital military medicine. We welcome this timely contribution and wish to offer a complementary perspective based on our experience within the French doctrinal framework of Medical Command and Control (MEDC2). In our view, the concept of a “medical director” represents a critical threshold: transitioning from technical execution to responsibility for organizing medical care and integrating it within military maneuvers. This transition demands much more than individual clinical skills—it requires the capacity to understand, anticipate, and influence the broader medical support system. It is also important to have a complete understanding of joint maneuvers and those of each specific environment (land, air, sea). Our MEDC2 model formalizes this progression by embedding medical direction within a command structure encompassing planning (MEDOPS), logistics (MEDLOG), intelligence (MEDINTEL), patient regulation (PECC), and advisory roles (MEDAD, MEDDIR). This doctrinal approach enables a diverse range of professionals—physicians, nurse officers, pharmacists, logisticians, and administrators—to exercise leadership responsibilities aligned with their roles and engagement levels.
