Abstract

In Current and Emerging Systemic Therapies for Advanced Clear Cell Renal Cell Carcinoma (ccRCC), the authors synthesize the therapeutic landscape of advanced ccRCC.1 They effectively trace the field's progression from VEGF-directed monotherapy to contemporary doublet immunotherapy (IO)–based strategies, highlighting the survival benefits in the CLEAR, CheckMate 9 ER, and KEYNOTE-426 trials. The review thoughtfully explores emerging modalities including HIF-2α inhibitors, AXL/TAM antagonists, radiopharmaceuticals, and oncolytic platforms. The authors emphasize the growing interest in perioperative systemic therapy, an area with ongoing trials that may improve long-term disease control for patients with biologically aggressive disease. However, if the cytokine era was “tic-tac-toe,” and the TKI era was “checkers,” the IO era could be considered “chess” because we will need to carefully weigh each systemic therapy exposure and its potential impact on subsequent treatment options. As IO-based regimens and treatment indications expand, the field must also prioritize efforts to identify patients at increased risk for immune related adverse events (irAEs). Predictive biomarkers for toxicity remain an unmet need, and the manuscript highlights understanding tumor microenvironment dynamics and metabolic reprogramming as a foundation for future work in this area. A strength of the manuscript is its attention to the need for improved risk stratification tools. As the authors note, molecular imaging modalities such as CAIX and PSMA targeted PET may enhance detection and characterization of metastatic disease. Emerging genomic, proteomic, immunologic, and imaging biomarkers show promise in refining prognostication.2 Epigenetic markers including DNA methylation patterns, microRNAs, and long noncoding RNAs represent additional avenues for risk refinement.3 Overall, the authors capture the rapidly evolving therapeutic landscape in advanced ccRCC and highlight areas of ongoing translational investigations. Integration of biomarkers in risk stratification systems, predicting and managing irAEs, and impending trial results investigating perioperative systemic treatments will shape the next era of personalized care for patients with advanced ccRCC.
