Abstract

A diagnosis of acute lymphoblastic leukemia (ALL) rarely relies on magnetic resonance imaging (MRI) or positron emission tomography/computed tomography (PET/CT).However, patchy, multifocal bone marrow (BM) disease, a rare manifestation of ALL, can be diagnostically challenging since marrow infiltration is not uniform and may potentially be missed during a standard BM biopsy without guidance from imaging modalities. In this article, we will review the course of a young adult male who presented with patchy multifocal BM disease due to relapsed B-ALL, ultimately discovered on MRIs of multiple extremities, and confirmed through multiple, repeated BM biopsies. His disease response to chemotherapy was monitored with PET/CT, and he ultimately underwent a bone marrow transplant (BMT).
