Abstract

INTRODUCTION: Pediatric reduced-port nephrectomy is associated with favorable cosmetic and postoperative outcomes. However, its application in the pelvic kidney has not been reported. MATERIALS AND SURGICAL TECHNIQUE: Two girls (age: 7 and 5 years) presented with continuous vaginal incontinence. Imaging studies, including MRI, contrast-enhanced CT, and retrograde vaginography, identified a hypoplastic pelvic kidney with a single ectopic ureter opening into the vagina. After placement of a ureteral catheter into the ectopic vaginal ureter, reduced-port laparoscopic nephroureterectomy was performed using multichannel single-port access devices placed at the umbilicus and one assist port in the lower abdomen. The hypoplastic pelvic kidney and ureter were removed laparoscopically, and the ureter was transected at the level of the broad ligament. DISCUSSION: The postoperative course was uneventful in both cases, and the urinary incontinence completely resolved. Reduced-port laparoscopic nephroureterectomy appears to be a safe and effective minimally invasive treatment option for removing a hypoplastic pelvic kidney and its associated ectopic ureter.
