APCU25OS23 Left atrial strain in hypertensive patient: diagnostic impact, applications and current status in the latest guidelines
Abstract
Introduction Hypertension is a major contributor to cardiovascular morbidity and mortality worldwide. Prolonged elevated blood pressure may cause structural and functional remodeling of the left atrium (LA). By far echocardiographic indices such as LA volume index (LAVI), are limited in detecting early diastolic dysfunction. Recently, Speckle tracking echocardiography is a method providing quantitative evaluation of LA reservoir, conduit, and contractile function. Hence, left atrial reservoir strain (LARS) has emerged as a sensitive marker for subclinical dysfunction. Our study aimed to investigate LA strain in hypertensive patients and discuss its diagnostic value with the introduction of the latest ASE 2025 guideline updates compared to 2016 recommendationsCase Presentation We conducted a cross-sectional study on 99 hypertensive patients and 100 healthy controls at Hue University of Medicine and Pharmacy Hospital. LA strain indices were obtained by 2D speckle tracking echocardiography and compared with conventional echocardiographic parameters of left ventricular (LV) structure and diastolic function. Results In hypertensive patients reservoir function from reference point at the beginning of the QRS and P wave are 32.71 ± 4.07%, 28.54 ± 3.43%, respectively. Conduit function are -15.78 ± 4.11%, -13.91 ± 3.83%. Contractile function are -16.90 ± 3.93%, 14.63 ± 2.98%. Left atrial strain is lower in hypertensive patient with normal left ventricle mass index and left atrial volume index compared to controlled group (p
