New Guidance on Iodinated Contrast Reactions | Podcast
Abstract
Dr. Lauren Kim-From the Radiological Society of North America, I'm Dr. Lauren Kim, and welcome to the Radiology Podcast. The most downloaded radiology article in 2025 is a work entitled, Management and Prevention of Hypersensitivity Reactions to Radiocontrast Media. This consensus statement from the American College of Radiology and the American Academy of Allergy, Asthma, and Immunology provides updated clinical guidance on the management of hypersensitivity reactions to iodinated contrast media. This consensus statement recommends that mild allergic reactions not undergo corticosteroid premedication, to premedicate for moderate reactions when the perceived benefits outweigh the risks of corticosteroid treatment, and to premedicate for severe reactions when the specific contrast agent is known, and to undergo skin testing when the specific contrast agent is not known. These recommendations represent a significant shift from conventional practice.Additionally, in all cases of reported previous mild, moderate, and severe contrast reactions, the consensus statement recommends switching the specific contrast agent when feasible to reduce the risk of recurrent allergic reactions.Taken together, these clinical guidelines present and appraise the strength of evidence regarding iodinated contrast reactions in order to standardize multidisciplinary care of patients who have a history of allergic reaction to iodinated contrast. It was my great pleasure to have on the podcast
