Abstract

Extract In their article published in this issue of the European Respiratory Journal, Rosen et al. [1] elegantly demonstrated that multi-beat pressure–volume loop-derived right ventricular–pulmonary arterial (RV–PA) coupling predicts transplant-free survival in patients with pulmonary arterial hypertension (PAH). Their findings reinforce the growing recognition that RV–PA coupling is a physiologically grounded and robust marker of disease severity and clinical worsening and – as first described by our group, to the best of our knowledge, and in the largest well-characterised PAH-only cohort reported so far – also of survival, thereby potentially complementing the traditional haemodynamic indices used for risk stratification [2–4].
