CYP51A1: A Bypass Gateway for Androgen Synthesis, and a Potential Therapeutic Target for Resistant Prostate Cancer
Abstract
A newly identified androgen biosynthesis pathway circumvents CYP17A1 via CYP51A1, offering a mechanistic explanation for the persistence of intratumoral androgens despite treatment with CYP17A1 inhibitors, such as abiraterone. Critical clinical questions remain regarding the role of this pathway in resistance to androgen receptor pathway inhibitors, the metabolic origins of the oxysterol substrates driving this “bypass” mechanism, and the potential of CYP51A1 as a therapeutic target.
