Abstract

Chapter 5 provides a brief overview of the most common hazard identification techniques, starting with a general introduction involving the “Swiss cheese model” to describe the concept of defence in depth. This is followed by details of the most common hazard identification techniques and at which stage of the project life cycle they are most appropriate. The most important techniques are then briefly described. Appendix A provides a simple example of HAZOP, a key technique in hazard identification. Finally, the chapter provides guidance on how collaborative studies such as HAZOP should be conducted—to ensure the best possible result by ensuring team members work together and avoid negative patterns of behaviour.
