Abstract

When a major healthcare AI system began recommending unnecessary surgical procedures across multiple hospitals, investigators faced a challenge unlike any in traditional digital forensics. The system had no obvious bugs in its code, its training data appeared clean at first glance, and its statistical performance metrics remained within acceptable ranges. Yet patients were being put at risk, healthcare costs were escalating, and medical professionals were losing trust in AI-assisted decision making.
