Abstract

Preventable adverse events are a major course of morbidity and mortality in healthcare settings. Traditional attitudes to quality improvement have focused more on ‘perfectibility’ of healthcare workers rather than systemic factors that affect human performance among individuals and teams. It is well recognised that failures in paratechnical aspects of care delivery (e.g. communication and teamwork) are major contributors to medical adverse events.
