Abstract

Handovers of patient care are known to be a significant source of risk of communication failures and harm regardless of nation or healthcare setting. To date, efforts at improving the consistency and reliability of handovers have largely used a “one size fits all” approach resulting in a standardized handover process. Not surprisingly, the impact of these initiatives have been mixed. This project describes a novel approach to the design of clinical handovers, using concepts from human factors and complexity science.
