Abstract

Despite near-universal implementation of manual counting protocols for surgical instruments and sponges, incidents of retained sponges and instruments retained surgical foreign bodies (RSFB) persist. Correct counts are reported in the majority of RSFB cases in published series, suggesting that negligence or failure to adhere to protocol is not a common etiology. Currently, there are no knowing methods of entirely eliminating the occurrence of RSFB.
