Developing Prospective Hazard Analysis in Healthcare: Evaluating the operating theatre environment using a new Toolkit
Abstract
Ward et al. ( 2010 ) describe the development of a new risk assessment tool kit for use in the UK&s;s National Health Service (NHS). The development of the Prospective Hazard Analysis (PHA) toolkit required the undertaking of a series of case studies. These studies provided significant insights as to the usefulness of the approach and also of the hazards and risks within the aspect for the system being studied.
