Abstract

Medication management is a critical component of healthcare and the most common source of error. 1–3 Adverse drug events (ADE) are a leading cause of mortality and morbidity and are very costly. 4,5 Medication management is an intensely independent highly complex cooperative task . 3 Lack of adequate drug therapy monitoring may be the most common cause of ADEs. 6 Information provided by each of the clinical roles is essential to positive patient outcomes. Little is known about the specific characteristics of the joint cognitive environment for medication management. 7 Communication of clear and explicit therapeutic objectives would likely enhance therapeutic control, improve medication reconciliation at transfers and help prevent, detect and manage ADEs.
