Risk factors in health services with multiple care providers
Abstract
This paper addresses the field of elderly patients who alternate between nursing homes and home care services. Their usual schedule of rotation is 3 weeks at home and 3 weeks in a nursing home. For the entire schedule, the patients remain within the responsibility of the primary health services.
