Abstract

Thrombosis, encompassing both venous and arterial events, poses significant clinical, economic, and humanistic burdens globally. This chapter highlights the crucial role of pharmacists in the evidence-based management of thrombosis. Through medication optimization, therapeutic monitoring, patient counseling, and adverse event management, pharmacists contribute significantly to improving clinical outcomes and minimizing complications such as bleeding and recurrence. Economically, pharmacist interventions reduce healthcare costs by promoting cost-effective therapies, minimizing hospitalizations, and ensuring efficient resource utilization. Humanistically, pharmacists enhance patients’ quality of life through education, psychological support, and improved treatment adherence. The chapter also explores current research evidencing the positive impact of pharmacist care and outlines facilitators and barriers influencing their role. Ultimately, integrating pharmacists more fully into thrombosis care pathways ensures a collaborative, patient-centered, and cost-effective healthcare approach.
