Abstract

This chapter explores the evidence-based pharmacist care approach in managing Type 2 Diabetes Mellitus, emphasizing its clinical, economic, and humanistic impacts. Pharmacist-led interventions, including medication therapy management, patient education, and multidisciplinary care, significantly improve glycemic control, reduce complications, and enhance adherence. These practices lower healthcare expenditures by minimizing hospitalizations and treatment costs. Moreover, pharmacist involvement improves patients’ quality of life through enhanced self-management support and satisfaction with care. Evidence supports pharmacists as integral members of diabetes care teams, contributing to better outcomes across all domains. Expanding their roles and collaborative practices is essential for optimizing diabetes management globally.
