Abstract

This chapter explores evidence-based pharmacist care in the management of Acute Kidney Disease (AKD), emphasizing its influence on clinical, economic, and humanistic outcomes. Pharmacists play a pivotal role in early detection, medication adjustment, prevention of nephrotoxicity, and management of comorbidities. Their interventions improve renal function recovery rates, reduce the progression to chronic kidney disease, and optimize overall treatment outcomes. Economically, pharmacist involvement decreases hospitalization durations and prevents costly complications. Humanistically, pharmacists enhance patient understanding, medication adherence, and engagement in care, resulting in better quality of life. Integrating pharmacists into AKD care teams supports safe, efficient, and patient-centered kidney disease management
